"Wrong reel/scene" errors

I once heard a story that WPGH/53 Pittsburgh would frequently show episodes of "The Flintstones" in which the middle part would run first, then the beginning, then the end. I don't recall seeing such incidents occurring.

When WTOG in Tampa Bay carried "The Flintstones", there was a couple of episodes that featured the announcer saying "The Flintstones, brought to you by...", then a couple of seconds of silence before the episode started in earnest. And another episode where, between scenes in a middle of the episode, featured an announcer saying, "We'll be back after a word from our alternate sponsor", before going to the next scene. (The commercial slots on WTOG's "Flintstones" were in different places than the network version.)
 
The Jeopardy incident is more likely to happen today in this digital world than it was in a video tape world. Syndicated shows are delivered to stations as one clip, with the different elements separated as sub-clips, (like songs on a CD). The broadcast day is now played back from a server by a playlist (like iTunes). If a board op edits the playlist and mistakenly inserts the wrong code for a show segment, like from a different day, the players on Jeopardy will change during commercial breaks.
