Release for playing local bands on radio?

Does anyone know what the text must say in a waiver to legally play a local band on a college, internet and cable only radio station? We sure could use it at Cal State San Bernardino (Coyote Radio). THANKS!
 
If the band gives permission (which most local bands will) then you can play their music. If they're on a label and you have a contract for paying royalties, then you can play their music.
